Custom Forms Quick Reference

Custom forms are groups of the custom fields you've created.  This allows you to select a subset of fields that you may want to show for a specific registration.  For example,  you may have a custom field called School that you use for registration for youth soccer but not adult soccer.  You would then have two forms called Soccer Youth Registration and Soccer Adult Registration. These forms could include any different fields, sometimes the same field sometimes different.

Creating

To create a form click the New Form button, enter a name and click Save.  Then drag the fields you want on the form from the Available Fields box into the Form Field box.  To remove a field drag it back to the Available Fields box.

Using For Registration

To use a form for registration go to the season and select which for you want to display at registration time.  You can test the forms out by adding someone in SAM or MySAM for that season.

Special Forms

There is one special form called Customer Profile Page.  Any field added to this form will now show up on the customer's profile page and can be changed and saved when editing the customer's profile.
